Policy on the Replacement of Risk Management Issued Passes

As on September 25th, 2018, NYSW Replacement Risk Management Passes will now cost $50 (Fifty Dollars).

 

ATTEMPTING TO RENEW A VALID EXISTING PASS WILL NOT BE HONORED.

PLEASE INFORM/REMIND YOUR STAFF NOT TO DO THIS.

 

Requests for replacement must come from a club officer and emailed to riskmanagement@nyswysa.org.

 

Exact Process for requesting currently valid replacement RM passes appears below:

 

1.) Request for replacement sent by email to riskmanagement@nyswysa.org. Request must come from a current club officer.  Requests from coaches will not be processed.

 

2.) We will research the pass status.  Once approved you will receive a payment link with authorization to proceed.  If there is an issue (IE: pass was taken during match, etc) we will let you know what to do.

 

3.) Replacement Fees are $50/pass pre-paid.  Passes are mailed regular first class as time allows.

 

DELIVERY: Overnight service is available at $40/per package and may take up to 48 hours to process (best to request replacements together rather separately unless they are going to separate locations).  Saturday delivery is contingent upon the carrier. Otherwise all shipments are standard first class USPS. Allow for 3-5 business days for delivery to your club.

 

NOTE:  ALL purchases are non-refundable.  Please make sure your staff member has exhausted all avenues of looking for their pass, including checking with past clubs.  DO NOT purchase the replacement UNTIL it is authorized.
