Four different authorization forms available:

• Permission to Travel for US Youth Soccer Sanctioned Events
• Tournament Guest Player form
• Permission to Participate in Another Organizations Event (Events Outside of US Youth Soccer)
• Friendly Games Hosting Agreement.

A reminder to all U09 and U10 teams: By policy adopted by the NYSW Membership on 11/18/2001, all U09 and U10 play in tournaments must be advertised/marketed as “Festivals” or “Jamborees.” Format of play must be round robin with no game results recorded/posted. There shall be no awards based on win/loss records for these age divisions—only participation awards for all players. Participation in tournaments that do not meet this criteria is a violation of NYSW policy and could result in your team being subject to penalties.  Travel to events that do not meet this criteria will not be covered by NYSW's insurance.

 It's easy to use.

1. Select which form you need from the menu and click on it.
2. Fill in the data online
3. After ensuring your data is correct, hit the submit button at the bottom of the form.
4. Your instantly approved form is returned and displayed.
5. Then use your browser's print function to print out your copy of the authorization or notification.
6. After you read and agree to the statements at the bottom, sign your form.

IF YOU ARE TRAVELING TO A US YOUTH SOCCER TOURNAMENT IN REGION 1, IT IS POSSIBLE THAT YOU DO NOT NEED TO DO THIS PAPERWORK. PLEASE READ THE REGION 1 TRAVEL POLICY BEFORE YOU COMPLETE YOUR TRAVEL FORM.

The Notification of Participation in Another Organizations Event should be used for those events that DO NOT have a US Youth Soccer Permission to Host. This includes all US Club Soccer events.
